Re: INSTRUCTIONS, Reports for 2007
Every credential holder must file an annual report most of which can now be done on line. Please follow these instructions carefully to complete your required report(s). Begin by selecting Report Forms on the denominational Website: www.emchurch.org
For Pastors in charge of EMC Congregations
Submit Forms M1946 & M1000
Have a copy of the last year’s statistical report for your church and a 2006 summation of the 12 monthly treasurer’s reports (Form F900) ready for use in preparing Form M1946. Prepare a one-page summary of your ministry ready to use in completing Form M1000.
Form M1946 is a pdf file and requires the latest version of the FREE Acrobat Reader program available here Acrobat Reader. After installing the program, find the M1946 form with your browser and wait for it to be accessed (be patient, it may be a long load). Fill out the form on line, being careful to answer all questions and fill in all spaces even if your answer is a 0 (zero). Print yourself a copy of it or save it to your computer for later use, then click on the SUBMIT button and follow the instructions to email the results to the conference authorities.
The STATISTICAL Section reflects your personal compilation of ministry data for the year. Fill in all spaces even if your answer is a "0" (zero) or "NO". The FINANCIAL Section is completed using the information from a compiled yearly treasurer’s report. The numbers in parenthesis refers to lines on the treasurer’s report Form F900. The amount on M1946, line 15, whether derived from tithing on income or applying the appropriate conference support formula, should meet or exceed the required amount.
Form M1000: This additional report is a record of your personal and church ministry for the year. It will be included as your personal report in the pre-conference materials and subsequent district conference minutes.. Concentrate on key accomplishments, plans for the New Year, and the ongoing denominational Missional Priorities. Upon completion, print yourself a copy and then submit Form M1000 on line.
For all other Elders, Admitted on Trial and Local Preachers
Submit Form M1001 on line. It will be included in the pre-conference report materials and subsequent district conference minutes.
For Superannuated or Supernumerary ministers, no report is required. However we would appreciate hearing from you of any ministerial service, activities or functions. Please use Form M1001
For Deacons or Deaconesses
Complete Form M1002 and submit it on line. It will be included in the pre-conference report materials and subsequent district conference minutes.
For All Credential Holders
Upon completion of your report, print out a copy for yourself, then date and sign it. Reports should be submited on line, thereby sending the results to the proper General and District Conference authorities with little effort. Alternatively, make paper copies to send by US mail to the Indianapolis Headquarters and the District Superintendent. Remember, If you are reporting on line, YOU MUST COMPLETE ALL REQUIRED FIELDS to successfully submit your report.
REMEMBER your report is due by January 31, 2008. This is critical for the General Conference.
